** Post season travel costs are not included**
Refund Policy:
All requests for refunds must be made, in writing, and received
by May 15th. All refunds will have $25
deducted for Administrative Costs.

1 Legible Copy of Birth Certificate- Proof of Original
2 Copies of 2023/24 Final Year Report Card on 8.5” x 11” paper
